RESUMO

Early upper gastrointestinal cancer screening skills teaching is the vacancy and difficulty of endoscopic teaching and training. In this study, a standardized training program was set up for endoscopic physicians who had mastered the basic gastroscopy techniques. The training period was 12 weeks, including theoretical training, early cancer awareness training, standardized gastroscopy operation technology, endoscopic diagnosis of early esophageal and gastric cancer, and theoretical and skill examination. After passing the examination, trainees can participate in early cancer case competition. According to questionnaire survey, 100%(15/15) of trainees thought that standardized training was necessary for improving the detection rate of early upper gastrointestinal cancer, and it could significantly improve the trainees' diagnosis level. Therefore, the overall setting of the standardized training process for the diagnosis of early upper gastrointestinal cancer in our center is generally reasonable, which is worthy of further promotion.

RESUMO

Adenosine 5'-monophosphate-activated protein activated protein kinase (AMPK), a heterotrimeric complex, is an important kinase to regulate glycolipid metabolism and energy balance involved in a variety physiological processes in human body. Many research indicated that the function and activity of AMPK were closely related to inflammation, diabetes and cancers. Recent reports show that inhibition of metformin (a first-line drug) on hepatic glucose in patients with hyperglycemia is associated with AMPK pathway, suggesting that targeting AMPK may be one of the effective strategies for the prevention and treatment of a variety of chronic diseases. Here, we review research progress on the structure, activation and regulation of AMPK in glycolipid metabolism to provide an insight into the basic and clinical research of diabetes therapy.

Objective@#To investigate the regulatory effect of faciogenital dysplasia 6 (FGD6) gene on hepatic stem cell differentiation.@*Methods@#FGD6 gene was selected for the co-intervention of target sequence, the AdEasy system was used for the construction of adenovirus vector and the packaging and multiplication of the recombinant adenovirus vector pSES-FGD6-siRNA, and the HP14.5 cells were infected. Immunofluorescence assay was used to measure the expression of FGD6 protein in HP14.5 cells, quantitative real-time PCR was used to measure the mRNA expression of FGD6, alpha-fetoprotein (AFP), and albumin (Alb), and Western blot was used to measure the protein expression of FGD6, AFP, and Alb. The empty pSES-Ad-RFP adenovirus vector was constructed as control in each group. All data were expressed as x±s, and a one-way analysis of variance was performed.@*Results@#FGD6 protein was mainly expressed in the nucleus of HP14.5 cells. The pSES-FGD6-siRNA adenovirus vector was successfully constructed and it downregulated the expression of FGD6 gene and the mRNA and protein expression of AFP in HP14.5 cells and upregulated the mRNA and protein expression of Alb (P < 0.01).@*Conclusion@#The inhibition of the expression of FGD6 gene in HP14.5 cells may differentiate HP14.5 cells into hepatocytes. Therefore, FGD6 gene plays an important role in the differentiation regulation of hepatic stem cells.

BACKGROUND:Preliminary data showed that the application of human platelet lysate to human umbilical cord mesenchymal stem cell culture can better maintain the characteristics of stem cells than the application of serum-free medium. However, the serum-free medium can better improve the proliferation of mesenchymal stem cells in vitro than the human platelet lysate.OBJECTIVE:To screen out a better mesenchymal stem cell cultivation system that can greatly maintain the characteristics and proliferation rate of human umbilical cord mesenchymal stem cells.METHODS:Six human umbilical cord specimens were inoculated in six culture systems, and the primary culture of umbilical cord mesenchymal stem cell was performed. These six culture systems were respectively MesenCult serum-free medium with 2% human platelet lysate (group A), StemPro serum-free medium with 2% human platelet lysate (group B), MesenCult serum-free medium (group C), StemPro serum-free medium (group D), low glucose-DMEM with 10% fetal bovine serum (group E), low glucose-DMEM with 10% human platelet lysate (group F). The cells were subcultured at 14 days after inoculation to compare the effects of different culture systems on the morphology, surface markers, differentiation and proliferation of human umbilical cord mesenchymal stem cells.RESULTS AND CONCLUSION:(1) The morphology of passage 3 cells in group D was elongated and uneven in size. The morphology of passage 3 cells was flattened in groups E and F, but the cells in the other groups were spindle-shaped and uniform. There were no significant changes in morphology and size between passage 3 and 5 cells in A and B. In group C, the morphology of passage 5 cells was more flattened and uneven in size compared with passage 3 cells. In group D, the morphology of passage 5 cells was more elongated than that of passage 3 cell. In group E, the morphology of passage 5 cells was more flattened than that of passage 3 cells. There was no significant difference in morphology between passage 3 and 5 cells in group F. (2) The expression rate of cell surface markers had no significant difference at different passages in each group. (3) The adipoinduction and osteoinduction rates were relatively higher in groups A and B compared with groups E and F, and lowest in groups C and D. (4) The cell proliferation rate for each passages in group A was significantly higher than that in group C. The cell proliferation rate for each passage in group B was significantly higher than that in group D. The cell proliferation rate for each passage in groups E and F was significantly lower than that in groups A and B. To conclude, these results suggest that the combination of serum-free medium with human platelet lysate could better maintain the characteristics and the proliferation efficiency of mesenchymal stem cells.

OBJECTIVE:To investigate therapeutic efficacy of bifidobacteria triple viable bacteria combined with infliximab in the treatment of moderate and severe ulcerative colitis(UC),and its effects on immune function. METHODS:Seventy-two UC pa-tients were selected from gastroenterology department of the First Affiliated Hospital of Chongqing Medical University during Jun. 2009 to Jan. 2015. They was divided into single group and combination group according to therapy plan,with 36 cases in each group. Single group was given infliximab 5 mg/kg,ivgtt,on 0,2nd,6th,8th week. Combination group was additionally given bi-fidobacteria triple viable bacteria 420 mg,tid. Both groups were treated for consercutive 2 months. Clinical efficacies were com-pared between 2 groups as well as the levels of humoral immunity and T cell function. The occurrence of ADR was observed in 2 groups. RESULTS:Clinical response rate of combination group was 86.11%,which was significantly higher than 52.78% of single group,with statistical significance (P0.05). After treatment,the levels of CD4+ and CD4+/CD8+ were significantly increased in both 2 groups,while the levels of CD8+,Th1 and Th2 were significantly decreased;the combination group was significantly better than the control group,with statistical significance (P0.05). After treatment,the levels of IgA and IgG were increased significantly in 2 groups,and the level of C3 was decreased significantly;the combination group was significantly better than the control group,with statistical significance (P0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Bifidobacteria triple viable bacteria combined with infliximab shows significant therapeutic efficacy in the treatment of moderate and severe UC, and can significantly improve humoral immunity function and the recovery of T cell level,with good safety.

RESUMO

Objective To investigate the clinical efficacy and safety of Jianwei xiaoshi oral liquid combined with mosapride on treating postprandial distress syndrome( PDS ). Methods Eighty cases patients with PDS diagnosed according to Rome-Ⅲ PDS diagnostic criteria were randomly assigned into treatment group and control group. Patients in treatment group were treated with jianwei xiaoshi oral liquid( 1 bottle/times,3 tiems/d,after meals)and mosapride,and in the control group were taken mosapride(5 mg/times,3 times/d, before meals)only for 4 weeks. After the end of treatment,the improvement of the symptoms,the total symptoms score,the total effective rate and adverse drug reaction were recorded. Results After the treatment,symptoms were relieved in both groups. The total symptoms score of the treatment group was(2. 90 ± 1. 39)scores,lower than that in before((11. 14 ± 1. 86)scores),and the difference was significant(t=22. 328,P﹤0. 01). The total symptoms score in treatment group was(2. 90 ± 1. 39)scores,significantly lower than the control group((4. 94 ±2. 03)scores;t =5. 217,P ﹤0. 01). The total effective rate was 87. 80%(36/41),66. 67%(26/39)in treatment and control group,and the difference was significant( t=5. 122,P﹤0. 05). There was no significant in terms of adverse drug reaction in two groups during the treatment. Conclusion Jianwei xiaoshi oral liquid combined with mosapride is a safe and effective treatment of PDS patients,which is better than using mosapride alone.

RESUMO

Based on the issue of medical English teaching, according to the immersion theory,we improved teachers' oral English, gave students a good foreign language circumstance, set students as the teaching center.to let them join the teaching activity and graspe medical English. All teaching reform worked well.
